Matt, I suffer from knee pain also just behind the knee cap in my left
knee. I have had two series of injection of synvisc and I'm going to try
it this winter without it. I think I've found a combination of stuff
that works to a fairly good degree.
1. I have custom orthotics that Michael Martindale did for me. Your doc
should be able to guide you to someone he thinks is good.
2. I stretch there often as your doc recommended, sometimes even
stretching in on the bike helps.
3. I do leg extensions to strengthen that small part of the quad on the
inside of the leg. I hold the extension out, muscles contracted, to
increase the strength in the desired area for a count of ten. I will be
working my way up to 3 sets of 15 at 140 lbs. Once strength begins to
increase in that small part of the quad, pain usually lessens somewhat
and by the time I get up to 125 lbs. I'm usually riding fairly pain free
with only minimal flair ups.
4. I take anti-inflammitories to control inflammation.
5. If the pain gets kind of bad, I go into an icing regime in which I'll
ice it twice a day deeply for several days. This usually will bring
things back to a manageable state.

I need some advice on orthotics (sp?). Late summer I started getting
pain in my left knee (anterior?). It has not gone away yet. I have seen
a specialist. He took x-rays and an MRI.
Diagnosis: He thinks that the problem stems with a fallen arch in my
left foot and lack of flexibility in my quads. He said the problem would
be worse if I were a runner. Apparently this can cause a lot of stress
in the tendons on the inside (just right of my left knee cap) of the
knee.
recommended solution: 1) regular routine of stretching quads. 2)
orthotics for my cycling shoes to support the arch.
I will probably try an inexpensive orthotic first to see if this does
the trick. The doc said I may have to consider custom orthotics if this
doesn't work. Any thought on who and where this can be done?
