Safety hazards in downtown/Hawthorne Bridge area

(Obviously, don't post if you don't think this belongs here). These are 'obvious' hazards unless you don't think of them, and then you're down:
1) the plates stretched across the Bus Mall road under construction downtown are really slippery when wet (so, duh, don't brake on them). I think they are on Madison and Main now, so going to or from the Hawthorne Bridge you might ride over one.
2) The repaving of SE 12th or whatever the road is that you take a right on to get from Hawthorne over to Madison if you're heading back to the Hawthorne Bridge from SE Portland has produced a rise of a few inches in the surface as you switch lanes. So you can maybe spill if you don't either hope over it or cross it at a good angle.
I went down braking on a wet plate at about 5 mph, and saw the rise at the last second and hopped over it. I put this on BikePortland.org too, and someone who saw it in Craigslist bike forum before it was flagged for being 'inappropriate' said they'd forward it to someone having something to do with the TriMet construction.
